Waiver/Release *

As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above-named minor participant(s). I agree on behalf of myself, my child(ren) named as minor participant(s) herein, or our heirs, successors, and assigns, to hold harmless and defend Highlight Catholic Ministries (doing business as Badano Sports), its officers, directors, employees, volunteers, representatives, and agents associated with the activities, from any claim arising from or in connection with my child(ren) participating in the activities, or in connection with any illness (including COVID) or injury (including death) or cost of medical treatment in connection therewith. I understand that some activities are conducted outdoors where weather conditions cannot be controlled and other activities may occur in facilities that do not have air conditioning. I warrant and agree that I will inform staff regarding any conditions at an event that are potentially unsafe for my child(ren). I understand participants not acting in a safe manner, improperly using equipment or facilities, causing damage or injury to property or other participants or failing to abide by program rules may be dismissed. I authorize Highlight Catholic Ministries to publish any photographs or videos taken of me and/or the minor participant(s) during these activities for use in marketing materials, including printed publications, social media, and websites. I understand no compensation will be given for the use of photos or videos. (Note: Names of those in the photograph/video will not be used unless additional authorization is granted.)
