Tryout Waiver and Release *

As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above-named minor participant. I agree on behalf of myself, my child named as minor participant herein, or our heirs, successors and assigns, to hold harmless and defend Highlight Catholic Ministries (doing business as Frassati Sports & Adventure and Badano Sports), its officers, directors, employees and agents, and any agency owning the facility, their employees and agents, volunteers, or representatives associated with the activities, from any claim arising from or in connection with my child participating in the activities, or in connection with any illness (including COVID) or injury (including death) or cost of medical treatment in connection therewith. I warrant and agree that I will inform staff regarding any conditions at the event that are potentially unsafe for my child. I understand participants not acting in a safe manner, improperly using equipment or facilities, causing damage or injury to property or other participants or failing to abide by program rules may be dismissed.

I hereby grant permission for my child to participate in lessons or tryouts with Highlight Catholic Ministries, and I acknowledge that I have carefully read and agree to the above Liability Waiver & Release.
